Maternity Leave Dates Notification Letter
This Maternity Leave Dates Notification Letter is designed for an employee
to respond to her employer’s Request for Maternity Leave Dates Letter. The
letter informs the employer of the date that the employee wishes to
commence her maternity leave.
Three optional paragraphs are included. The first is for use by an employee
that only wishes to take Ordinary Maternity Leave (“OML”); the second is
for an employee that wishes to take all 52 weeks of OML and Additional
Maternity Leave (“AML”); and the third is for an employee that wishes to
take OML and only part AML.
This letter should reach the employer by the fifteenth week before the
Expected Week of Childbirth.
