This Request for Maternity Leave Dates letter acknowledges the employee’s notification of the Expected Week of Childbirth and requests the date on which the expectant mother wishes to commence her Ordinary Maternity Leave (OML) and Additional Maternity Leave (AML).

The employee’s reply should be made by the fifteenth week before the EWC.

Female Employees are entitled (subject to various notice requirements) to 26 weeks’ OML and to return to work without giving notice of the date on which they intend to return. If they intend to return early, eight weeks’ notice must be given. A woman is also entitled to AML of a further 26 weeks commencing immediately upon the end of OML.

The employee is not allowed to work within two weeks of birth (or four weeks if she works in a factory).
