Sickness and Absence Self-Certification Form

Absence Self-Certification Form is used by an employee to record a period of sickness absence and give the employer the relevant details.

It is intended for short-term sickness absence and should be completed in line with the employer’s Sickness and Absence Policy.

When this self-certification form should be used

This form should be completed for sickness absence of up to seven calendar days.

The period recorded should show the employee’s actual days of sickness, even where those days include weekends or public holidays.

Return to work and fit note requirements

• The company may hold a return to work interview to discuss the absence and illness.
• Sickness absence of more than seven calendar days will normally require a Fit Note or Medical Certificate.
• Further Fit Notes or Medical Certificates should be provided as needed to cover the full period of absence.
