Pro-forma Reply from Shareholders re Full Names

This pro-forma reply slip is a letter of response addressed to the company from a shareholder either confirming or correcting the information that a company has in relation to a shareholder’s full name and service address.

This reply slip should be sent to each shareholder at the same time as the letter explaining the new requirements of the Economic Crime & Corporate Transparency Act for full names of shareholders to be included on company registers, including the duty on members to notify this information to companies.

Requisite details should be inserted into the highlighted fields, or the wording can be adjusted to suit your purposes. Wording in square brackets is optional. If it is required, then the brackets should be deleted and the wording within them retained. If the wording is not required, then it can be deleted.
