Staff Performance Appraisal Form: Guidance Notes For Appraisee
The staff appraisal process should not be seen as a means of solely checking "productivity". It should be used to identify strengths and weaknesses of staff, training needs and aspirations, and achievable targets as an integral part of staff management and motivation.
The Appraisal form is divided into two parts, each with Guidance Notes.
Part I should be completed by the Appraisee. These Notes explain the scoring system and what is expected from the Appraisee in each of the following headings:
1.Duties & Responsibilities
2. Discussion Points
3. Objectives (prior year) and performance
4. Capability & Knowledge
5. Objectives/Targets for next 12 months
6. Data Protection
This document has been updated in accordance with the requirements of The General Data Protection Regulation (GDPR), which requires employers to comply with principles for processing personal data, including protection against unauthorised access of personal data.
