This letter is used to confirm the outcome of a formal meeting held under a company’s long-term sickness absence management procedure.

It follows the Invitation to a Stage 2 Long-term Sickness Absence Meeting and is used where the employee has reached the relevant level of absence set out in the employer’s absence management procedure.

What this Stage 2 long-term sickness absence outcome letter confirms

The letter records the key points discussed at the meeting and confirms that the employee has been issued with a second formal warning under the absence management procedure.

It also confirms that personal data will be collected in accordance with the Company’s Data Protection Policy.
