Paternity Leave Request Form (Birth Child)
This form should be used in order to give employees the means to request paternity leave and pay in respect of a birth child.
Employees are entitled to take one week’s or two weeks' paternity leave. If the employee takes two weeks of leave, they can choose to take them together or separately.
In order to be eligible for paternity leave, the employee must be taking time off to look after the child and be:
- the father of the child;
- the husband or partner of the mother (including same-sex partners);
- the intended parent (if the employee is having a baby through a surrogacy arrangement).
Employees are eligible for paternity leave from the first day of employment.
