This Request for Shared Parental Leave to be given by parent who wants to take a period of shared parental leave form should be used by employees to provide a period of leave notice to take Shared Parental Leave. The new system of Shared Parental Leave (SPL) has been introduced in respect of children due on or after 5 April 2015 or due to be adopted on or after 5 April 2015. In order to take SPL, an employee must give the Company with a written notice setting out the start and end dates of each period of SPL requested in that notice.

A period of leave notice must be given not less than eight weeks before the start date of the first period of SPL.

The notices that parents must give to an employer are:

• A maternity leave curtailment notice;
• A notice of entitlement and intention; and
• This request for shared parental leave.
