This Business Information Form is designed to help sole traders and partnerships providing services comply with the Provision of Services Regulations 2009 by supplying key business information to customers in one document.
It is intended for use alongside standard terms and conditions or a contract. Customers must be made aware of the information in this form before they enter into any contract.
When to use this business information form
The Regulations require certain information to be given automatically to customers and certain other information to be provided only if requested.
This form covers the information that must be provided as a matter of course. The separate Requested Information Form should be used for information that only has to be supplied on request.
What this form covers
Many service providers will already include much of the required information in their usual customer documents. This form provides a simple way to bring the core mandatory information together in one place.
It covers:
- business and contact details;
- registration details;
- after-sales guarantee information;
- VAT details;
- insurance information;
- complaints information and contact details; and
- dispute resolution information.
What is not included
This form and the Requested Information Form do not contain all information required under the Regulations. Any omitted information should usually be included in other documents such as an estimate, quotation, or contract.
Business type and related documents
This version is designed for sole traders and partnerships. It leaves out details that are relevant only to companies and LLPs, for which a separate form is available.
It should generally be used together with the Requested Information Form where you need to meet both categories of disclosure required by the Regulations.
