Business Information Form (Sole Trader or Partnership)
Under the Provision of Services Regulations 2009, businesses providing services to their customers are required to provide certain specific information about their business to customers.
Many businesses will already supply most, if not all, of the information required under the Regulations.
This Business Information Form is designed to enable all service providers to easily comply with their obligations under the Regulations with one simple document.
This form and the Requested Information Form do not contain all information required under the Regulations, however that which is omitted should be included in other documentation such as an estimate, quotation or contract as a matter of course.
The information set out in the regulations falls into two categories – that which must be provided to customers and that which must be provided on request. This document caters for the first category. A further form, (also available in this subfolder), should be used to provide information in the second category.
This template is designed for use by sole traders and partnerships. It omits certain details that are relevant only to companies or LLPs. An alternative form has been created for such businesses.
This document and the Requested Information Form are both designed for use as an attachment to standard terms and conditions of business or a contract. In either case, it is important to note that customers must be made aware of the information contained in these forms in advance of entering into any contract.

Once you have subscribed to the appropriate document folder click on the “Download Document” button below. You will be asked what you want to do with the file. It is recommended that you save the document to a location of your choice prior to viewing.
