This Requested Information Form is designed to help service providers comply with the Provision of Services Regulations 2009 by supplying information that must be provided to customers on request.

Many businesses will already provide some or all of this information in their usual customer documents, but this form brings the key requested disclosures together in one place.

It is intended for use alongside standard terms and conditions or a contract. Customers must be made aware of the information in this form before they enter into any contract.

When to use this requested information form

The Regulations divide the required information into two categories: information that must be provided automatically and information that must be provided only on request.

This form is designed for the on-request category. It sits alongside the Business Information Form, which covers the other category of disclosure.

What this form covers

This Requested Information Form covers the additional information that customers may ask for under the Regulations, including:

• pricing and price calculation;
• regulated profession details;
• other business activities; and
• codes of conduct.

What is not included

This form and the Business Information Form do not contain all information required under the Regulations. Any omitted information should usually be included in other documents such as an estimate, quotation, or contract.
