Trustee Declaration of Eligibility Registration Form


This Trustee Declaration of Eligibility Registration Form is a model form produced by the Charity Commission. In this form the trustees confirm to the Commission that they are eligible and willing to act as trustees. It is for use, and must be used, from 1 October 2018. From that date the Commission will only accept this form, and the Commission's previous version of the form ceases to be valid. 

When a charity submits its application to the Commission to be registered as a charity, the Commission requires the application to be accompanied by this form. (Only if a charity’s annual income does or will exceed £5,000 will it need to register with the Charity Commission.)
