RIDDOR Form F2508 - Report Of An Injury Or Dangerous Occurrence

Death and certain injuries or accidents at work must be reported under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR) 2013.

The employer or other person with responsibility under RIDDOR must keep a record of all over three day injuries, if the incapacitation is over seven days (not counting the day on which the accident happened) then the over seven day injury must be reported.

Under RIDDOR 2013 in Great Britain, most reportable incidents require notification without delay, with the report received within 10 days of the incident. This includes work-related deaths, specified injuries and dangerous occurrences. Injuries resulting in a worker’s incapacitation for more than seven consecutive days must be reported within 15 days of the incident.

Use the appropriate HSE online reporting form. Fatal accidents and specified injuries to workers can alternatively be reported through HSE’s telephone reporting service. Completing or retaining a copy of this template does not itself fulfil the reporting requirement.
