An Internal Accident Investigation Report is required in cases of serious injury, and certainly any injury that needs to be reported under RIDDOR, and should be used to verify and provide greater detail on the accident as well as investigate the adequacy of the Health & Safety procedures, controls and precautions.
This Internal Accident Investigation Report can be further supported by completing the Single Hazard Risk Assessment form.
