Carer’s Leave Request Form

From the start of their employment, employees are entitled to take unpaid leave to give or arrange care for a dependant who has:

• A physical or mental illness or injury that means they are expected to need care for more than three months
• A disability
• Care needs because of old age.

Employees can take up to one week of leave every 12 months.  This leave can be taken as a whole week or individual days or half days.

Carer’s leave is different to time off for dependants leave, which is for emergency situations.

This Carer’s Leave Request Form can be used by employees to request carer’s leave and includes a declaration by the employee confirming their entitlement to carer’s leave.
