This sub-folder contains general risk assessment forms that can be used to help you to comply with your obligations under health and safety law. In addition, correctly using the forms to carry out the risk assessment process should enable you to make your workplace a safer, healthier place to be.
The Standard Risk Assessment Form is a template that can be used by the duty holder to record details of precautions, controls, training, instructions, the provision of information, systems and procedures, and identify related hazards, potential hazards and risks. It should be used for specific locations on business premises. Risk assessment must be appropriate ("suitable and sufficient") to the nature and scale of the work, the likelihood of hazards, the extent and severity of the risk(s), and be understandable and repeatable.
The Standard Risk Assessment Form and Risk Assessment Action Plan work in combination with the detailed set of “Guidance Notes – How to do a Risk Assessment”. The Risk Assessment Guidance Notes provide detailed guidance for duty holders in how to carry out a risk assessment and fill out a risk assessment form and also detail the 5 steps to risk assessment recommended by the HSE.
