Single Hazard High Risk Assessment Form

This Single Hazard Risk Assessment form should be used to evaluate, recommend actions required and record responsibility in respect of hazards and risks identified by Risk Assessment Forms, Audits and Questionnaires, or those identified by occurrences in the workplace. The Single Hazard Risk Assessment form should then be managed and monitored by the relevant Risk Assessment Action Plan.

This form contains the following sections:-

1. Description/Identification of the hazard/risk
2. Consequences - description of the harm that might be caused by the hazard/risk
3. Identification of the persons at risk, and the level of risk
4. Consideration of precautions and controls
5. Record of the form's inclusion in the relevant Risk Assessment Action Plan

Small, office-based businesses are generally low risk environments and our Standard Risk Assessment Form and Risk Assessment Action Plan will normally enable an employer to manage, record and review the risk assessment process. However, in the event of an accident or identification of a high risk hazard it may be appropriate to record and monitor that risk on a separate form.
