If the accident requires reporting then this should be noted on the Accident Record and this report must be sent to the Incident Contact Centre (details provided on the form) or your local HSE office or local authority within 15 days of the accident.

In respect of diseases, if you or any of your employees is told in writing by a doctor that you/he/she is suffering from a reportable disease you must send a completed F2508A to the relevant authority immediately.
