These Guidance Notes on how to carry out a Risk Assessment in your workplace will help you or your assessors carry out risk assessments, especially if you are not used to doing them.

The guidance is based on the Five Steps to Risk Assessment recommended by the HSE:

1. Identify the hazards;

2. Decide who might be harmed and how;

3. Evaluate the level of risk arising from the hazards you have identified and decide whether the existing precautions are adequate or you need to add more;

4. Record your findings;

5. Review your assessment and revise it if necessary.

These Guidance Notes On Completing Risk Assessments explain how to use the Risk Assessment Form and the Risk Assessment Action Plan, including some worked examples.

This document also explains the application of the probability and severity evaluation of hazards and potential hazards identified during the risk assessment, and the use of priority ranking to help manage the risk assessment review process.

The guidance notes also refer to the potential risks of not carrying out proper risk assessments, including associated fines and information on the Court’s sentencing guidelines.
