Notice of Change of Landlord Address Form RHW4

This Notice of Change of Landlord Address Form RHW4 is prescribed by the Welsh Government.  The form should be used by Landlords to notify contract-holders when there is a change of address for service of documents on the Landlord.  The prescribed form RHW4 must be used to ensure compliance with s39 Renting Homes (Wales) Act 2016 and be issued within 14 days of the change of address.
