This Donations - Due Diligence Questionnaire template is for use by trustees of a charity to help it assess whether it would be appropriate for the charity to, as the case may be, either accept or refuse a proposed donation from a particular donor. 

The Commission has a detailed guidance note on  “Due diligence, monitoring and verifying the end use of charitable funds” which can be found on the Commission’s website here. 

It includes guidance on the topics of “Know Your Donor” and refusal or return of a donation by a charity.
