The Companies House Form 12 is a declaration that all the legal requirements (per Section 10 of the Companies Act) for the incorporation of the company have been complied with. That is, that the details per form 10 are correct. It can be signed by a Solicitor involved in the formation of the company or by one of the persons named as a director or the secretary.
The Declaration on Application for Registration must be signed in front of a Commissioner of Oaths, a Public Notary, Justice of the Peace or a solicitor (other than a solicitor involved in the formation of the company, if applicable).
