Confined Spaces Permit-to-Work Form

A permit-to-work is a formal recorded process used to control work which is identified as potentially hazardous. The objective is to ensure that proper consideration is given to the risks of a particular job. 

A confined space is a place which is substantially enclosed (though not always entirely), and where serious injury can occur from hazardous substances or conditions within the space or nearby (e.g. lack of oxygen).

Some examples of confined spaces include storage tanks, silos, enclosed drains and sewers, and poorly ventilated rooms.

The Confined Spaces Permit-to-Work form can be used for all operations working in or entering into a Confined Space, and should be signed by both the company and the contractor responsible for carrying out the work.

This Confined Space Permit template is in fixed field format and contains macros. All macros are virus free. Always click "enable" if an automatic pop-up appears.

Simply press TAB to jump from one field to the next and SHIFT + TAB to go back. Alternatively, use the mouse to click from one field to the next.

The template can be unlocked by clicking on the "Padlock" icon on the tool bar.
