This Notification and Request for Paternity Leave is designed for use by Expectant Fathers to inform their employers that their partner is Pregnant and when the baby is due. It includes a request for Paternity Leave and Paternity Pay. In addition this letter provides the necessary declaration of paternity for requesting Statutory Paternity Pay, also known as a self certificate.

Upon receipt of this letter the employer should respond with a letter either approving or denying the request for paternity leave (EMP.PAT.03 or EMP.PAT.04.)
