This Job Application Form template complies with the Equality Act 2010.

It is considered best practice to exclude such information such as Date of Birth, and dates in relation to previous employment from Employment Application Form(s). Candidates are requested, however, to provide information about material gaps in their employment history.

The Personal Details section of this Job Application Form template has also been amended so that there is no direct request for information that might distinguish candidates on the basis of sex.
