Employers should ensure that all their employees are treated equally irrespective of race, sex, sexual orientation, religion, disability, age, gender assignment, marital status or ethnic origin for training purposes.

The Equality and Diversity / Equal Opportunities Training Monitoring Form should be provided to the employee when the employee is applying for training or prior to commencement of the training.

The information from this form should then be entered into the Equal Opportunities Training Monitoring Control Sheet.

This form contains the following sections:-
1. Gender
2. Preferred Title and Full Name
3. Marital Status
4. Ethnic Origin
5. Disability

6. Age Range

This template is in fixed field format. Simply press TAB to jump from one field to the next and SHIFT + TAB to go back. Alternatively, use the mouse to click from one field to the next.

The template can be unlocked by clicking on the "Padlock" icon on the tool bar. To display the “Padlock” icon click on “View” and then “Toolbars”. From the “Toolbar” menu enable “Forms”.
