This Disability Access Audit Form - Blank Format can be used when a particular access issue is not covered by one of the 17 specific Disability Access Audit Forms provided in this subfolder. This form is designed so that you can insert the relevant specific issues, technical tips and identification of actions to be taken. This will assist you in recording your observations and identifying potential problems. If any action is required this should be noted and then also recorded on the Disability Access Audit Action Plan.

These issues are not exhaustive. They can be deleted or expanded upon depending on your building - see instruction below.
