This Trustee Confirmation Eligibility and Responsibility Registration Form CC5c is a model form produced by the Charity Commission. In this form the trustees confirm to the Commission that they are eligible and willing to act as trustees.

When a charity submits its application to the Commission to be registered as a charity, the Commission requires the application to be accompanied by this form CC5c. (Only if a charity’s annual income does or will exceed £5,000 will it need to register with the Charity Commission.)

Before completing Form CC5c, the trustees should each complete and sign a Trustee Declaration of Eligibility Form CSD1382 which can also be found in this Sub-folder. CSD1382 is for the charity’s own records and is not to be submitted to the Commission. The trustees of the charity will then need to complete, sign, and then submit this Confirmation Form CC5c to the Commission with the charity’s application for registration.

This Confirmation Form, CC5c will list all of those who will be the initial trustees of the charity upon registration. The charity should not use this form for any other purpose. At any time subsequent to registration, whenever any trustee resigns or a new trustee is appointed, the charity must notify the Commission online or in the charity’s annual return of that change. There is no form prescribed by the Commission for notification of such changes to the trustees.
