Fire Safety Multiple Hazard Risk Assessment Form

This Fire Safety Risk Assessment form is a compact or short-form document. It has been updated to comply with the no smoking regulations effective from 1st July 2007. The MultipleFire Risk Assessment Form is appropriate to businesses where potential hazards and numbers of employees & visitors are few, and the extent and severity of risk limited. This MultipleFire Risk Assessment form is only suitable for single room offices.

Following the implementation of the Fire Regulations in October 2006 businesses must carry out their own Fire Risk Assessments. Accordingly, it is recommended that our more comprehensive set of fire safety risk assessment documents are applied:
- Fire Safety Risk Assessment Form and Checklist
- Fire Hazard Risk Assessment Form
- Fire Safety Risk Assessment Action Plan

The Fire Safety Risk Assessment form and Checklist provides a comprehensive (but not exhaustive) list of questions to enable the identification of potential hazards, a detailed risk assessment should then carried out and recorded on the Fire Hazard Risk Assessment Form for each hazard, with all the actions planned on these risk assessment forms then monitored and managed on the Action Plan.

This MultipleFire Safety Risk Assessment Form does, however, provide a simple format to comply with the "5 Steps to Risk Assessment" recommended by the HSE:
1. Look for and identify the hazards;
2. Decide who might be harmed;
3. Evaluate the level of risk(s) arising from the hazards and decide whether existing precautions are adequate or more should be done;
4. Record findings;
5. Review assessment(s) from time to time and revise if necessary.
This Fire Safety Multiple Risk Assessment Form should be tailored to be "suitable and sufficient" for the nature and size of the operation(s), the likelihood of hazards and the extent and severity of the risk. 

This Fire Safety Risk Assessment Form provides for the recording of the following:
1. General and responsibility information
2. Description of hazards
3. Consequences of hazards
4. Risk levels
5. Persons at risk
6. Controls and precautions in place
7. Further action required
8. Review timetable.

Sections 2 - 7 have example wording. These examples are not exhaustive but provide an indication of the considerations and answers required to carry out the risk assessment.
