An accident book should be provided in the workplace to record injuries that happen to employees and should be used to record such information as the date, time and place of the accident/ incident; name and job of the affected person; details of the injury and what treatment was given; what happened to the person afterwards; and name/signature of the first aider.
This accident record book is one of many legal documents that should be kept by the Duty Holder. In the event of an accident the injured employee or someone acting on their behalf should complete an accident record. Blank accident records should be available to all employees. The completed accident records must, as required by Data Protection legislation, be kept securely by the Duty Holder. This accident report book should be used to monitor and control the individual accident reports.
This accident report book template is in fixed field format and contains macros. All macros are virus free. Always click "enable" if an automatic pop-up appears.
When the accident record book template is open, simply press TAB to jump from one field to the next and SHIFT + TAB to go back. Alternatively, use the mouse to click from one field to the next.
