Blank Method Statement Form

Method Statement - Blank Form

This blank site task method statement form will allow you to write a safe system of work to minimise the risks to your employees while they are carrying out any task.

If you are new to method statements you may find it useful to use this blank method statement in conjunction with one of the worked examples in this folder and the guidance notes on how to write a method statement. The form is designed with some sections to be filled in with the detail relevant to your particular location and other sections, such as the box entitled ’PPE required’, contain a non-exhaustive list of suggested options and should be deleted or added to as appropriate. Each method statement must always be site specific.

When the method statement has been completed and the methodology has been agreed, then that agreed method statement becomes the document that must be worked to on site. The site workers should read and sign the method statement to show that they understand it and agree to work in accordance with it; the final page includes a section for each site worker to sign.

This blank method statement has been professionally drafted and can be used either as a template or can be printed and filled in on the relevant site while the assessment is being carried out.
